Office Policies

Confidentiality
Your medical information is strictly confidential. We will not release it to anyone without your written consent. That includes phone calls we may receive from family members or relatives regarding your appointments or visit outcomes. A family member may, however, accompany you to your appointments if you wish. An authorization form must be signed if you want a copy of your medical records sent to another provider.

Prescriptions and Refills
If you need a prescription refill, please call during office hours so your chart can be reviewed and documented. We discourage prescribing new medications over the telephone as only an office visit will assure an accurate diagnosis.

Medical Records
You need to sign a whenever you ask for a copy of your medical records regardless of whether it is for personal use or for another provider/second party. Please be as specific as possible when indicating what information you need copied. General information on your chart is:

>  X-ray and lab reports
>  OB records
>  Progress notes
>  Operative report
>  Hospital notes
>  Discharge summary
>  Diagnostic test results (scans, etc.)
>  Biopsy reports

Appointment Information
Women’s Healthcare of Monroe is always open to new patients. Appointment times vary. Please call the main office number at (318) 323-2244 to schedule an appointment at any of our locations.
We try to see you as close to your scheduled appointment time as possible and ask that you arrive 10-15 minutes early to complete any necessary forms or to update information. At your first appointment with Women’s Healthcare of Monroe, we ask that you bring a photo ID and your insurance card. Also, please bring your insurance card to every appointment after your first to verify no changes have been made
Running late?
As a courtesy to the other patients waiting to see your provider, please reschedule your appointment if you will be late by 10 minutes or more.
Need to cancel an appointment?
Please call our office as soon as possible to cancel your appointment. Not showing up disrupts Dr Armstrong’s schedule and the unfilled slot is a lost chance for another patient to be helped.
We respect that your time is valuable too and appreciate your understanding and cooperation.
Urgent After Hours
The billing office and most of our administrative staff can be contacted during our business hours:

Monday-Thursday – 8:00 am to 5:00 pm

Friday – Closed

Saturday-Sunday – Closed

We are closed for all major holidays:

>  New Year’s Day
>  Memorial Day
>  4th of July
>  Labor Day
>  Thanksgiving Day and the Friday after
>  Christmas

Urgent situations after hours:In case of emergency, call 911
